Rebates can be submitted to ACTIVE in one of two ways:
- ACTIVE Advantage Members can fill out the online form on this page including:
- Member name
- Mailing address for the check to be sent
- A copy of your receipt
- A copy of your photo showing off your accomplishments!
- ACTIVE Advantage Members can forward, on the same day, their receipt emails to advantagerebates@activenetwork.com.
All submissions must include the ACTIVE Advantage member’s name, email address, and mailing address where the rebate check is to be sent.
